CVSN Team Registration Form
This is merely a form to be filled out by club coach, director or founder. Proof of Connection to Applied Club will be verified before info is to be adjusted, corrected, or added. This Registration Form is for Stater Team Profiles who are not yet showcased and would like to be present on the local club page. We will limit Club Page Managers to 2 People per club max to limit to confusion.
